SUMMARY OF SERVICES
CONSULTATIONS: An initial meeting with a geriatric care manager in our office can provide the
family with an opportunity to explore various options, and their costs, available in the community.
ASSESSMENTS: Includes a thorough psycho-social evaluation as well as information about the
resources and options available in the community. Financial planning and eligibility for programs
are also discussed. These assessments are often provided free of charge.
CARE MANAGEMENT: Includes client visits, long-term planning, consulting, family conferences,
telephone consultations, review of financial, legal or medical issues, referrals to geriatric
specialists, liaison to families at a distance.
MONITORING/OVERSIGHT: While you are out of town, geriatric care managers are on call 24 hours
a day, every day. Staff can be available on an "as needed" basis or can schedule regular visits,
providing professional, knowledgeable oversight in your absence.
FACILITY REFERRAL SERVICE: At no charge, our staff refers individuals to a wide variety of housing
options, including foster homes, assisted living facilities, residential care facilities and nursing homes.
MONEY MANAGEMENT: Includes bill paying assistance, money management, insurance analysis and review,
financial planning for Medicaid, and Power of Attorney or Trustee responsibilities.
GUARDIANSHIPS: When intervention is needed to protect an incapacitated person, ElderCare Resources Inc. serves
as a court appointed guardian, either on a temporary or permanent basis.
CONSERVATORSHIPS: Also a court appointed position, we provide financial management to ensure
protection of income and assets.
