ElderCare Services

CONSULTATIONS and ASSESSMENTS
An
ElderCare Resources Care Manager will explore needs and options with the family or client at an initial meeting. A consultation takes place in our office. An assessment involves a home visit. After evaluating the situation medically and financially, we will recommend a course of action, appropriate living arrangements, and care services. ElderCare Resources can refer clients to trusted local professionals, such as attorneys and physicians, if needed.

CARE MANAGEMENT and PATIENT ADVOCACY
The nurses on our staff can help manage a crisis or chronic health problems. As needed, a nurse care manager will visit the client at home to set up medications or monitor health conditions. The care manager communicates the patient's needs to doctors, dentists, and other medical providers.
ElderCare Resources staff also can schedule caregivers, routine and specialist doctor appointments, and transportation to appointments. Any other household management needs can be addressed, as well.

CLIENT MONITORING
When the family caregiver is away,
ElderCare Resources staff will check up on the client and manage any needs or emergencies. We're on call every day, all day and all night. We can schedule regular in-home visits or telephone check-ups.

MONEY MANAGEMENT
Includes bill-paying assistance, checkbook management, insurance analysis and review, and information about Medicaid planning.
ElderCare Resources staff can serve as Power of Attorney, Trustee, Representative Payee, or Estate Administrator. For clients with significant assets, we partner with certified financial planners to establish an appropriate investment plan.

GUARDIANSHIP and CONSERVATORSHIP
ElderCare Resources staff serves as a Court-appointed guardian when a person cannot make competent decisions. As Court-appointed conservator, ElderCare Resources protects and manages an incapacitated person's assets and income. Karen Fabiano, MPAff is a National and Oregon Certified Guardian.

FACILITY REFERRALS
ElderCare Resources staff can select from the wide variety of care facilities, assisted living facilities, and foster homes in our area to refer seniors and families to appropriate housing options.
